- Middle English translation, comprising thirty chapters selected from eight books of the Latin version, itself a translation from the original Old Swedish.
- Script: Anglicana.
- Decoration: The first chapter has a 7-line blue initial. Two-line blue initials begin the rubric to the first chapters and all subsequent chapters. Rubricated throughout.

- France (?), late 16th or early 17th century. Brown calf over pasteboard. In the middle of the upper and lower covers is a gold-stamped oval medallion that depicts a Crucifixion scene, with the Virgin Mary and St. John standing beside the cross.

- Garrett MS. 145 is of unknown early provenance. No Middle English manuscript of the Revelations was recorded in the library of Syon Abbey (Ilseworth, Middlesex). Intermediate ownership is unknown. Robert Garrett (1875-1961), of Baltimore, Maryland, Class of 1897, purchased the manuscript from the London antiquarian bookseller Bernard Quaritch on 1 October 1905 and wrote “R.G. Oct. 1905” in pencil on the front pastedown. Garrett deposited the manuscript in the Princeton University Library in February 1906 (deposit no. 1397). His gift to the Library, 1942. There is an unidentified note “Old No. 894” on the third front flyleaf.
